Chronic Care Management Nurse

DescriptionThe Chronic Care Management Nurse works as part of the clinic provider team, assists the provision of primary health care, provides direct patient services, and performs specifically designated independent procedures.
The Chronic Care Nurse will plan and coordinate chronic disease management as part of a multidisciplinary team.
The Chronic Care Management Nurse will work with providers, clinical staff, and other Coordinated Care services to develop a comprehensive service delivery system aimed at meeting the needs of the patients/clients.
The Chronic Care Management Nurse will also be responsible for managing patients enrolled in Remote Patient Monitoring.
Other duties include Transitional Care Management.
Must have a current RN license and CPR.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
